If you have an "Ideal Standard" Cistern , it will fit. Most cisterns require a wider bore.

How do i remove the small screw, its all rusted over

No easy answer, I've had them sheer off when too much force is applied. You might try a liberal spray of WD40 but give it time to soak in and give it quite a few decent taps to encourage the WD40 to trickle in between the threads. Also try some WD40 on the shaft of the lever to encourage movement. You may be able to slide it off. I'm afraid elbow grease and persistence is the most likely requirenent though.

To fit this do you need to turn off the water

There should be a valve on the toilet water inlet pipe that you can turn off with a screwdriver. Then flush the toilet which will empty the water in the cistern. Then you can change the handle without any water issues

what size is the handle i needit to be less than 4 inchs

The overall length of the handle is around 100mm circa 4 inches
